In order to receive your CE credits for the QCHF DON Conference, you must complete the attestation of attendance and an online session evaluation. Visit www.cahf.org and log into your Member Dashboard. Follow the instructions to complete the attestation and evaluate the conference. Then, download and/or print your certificates.


Attestations and evaluations will be available once the conference has concluded and must be completed by July 5, 2022.


Please follow the instructions below starting at your member dashboard.

  1. Take the Attestation
    1. In your member dashboard
    2. Scroll down to “Events” click on “QCHF Learning Center”
    3. Choose the Conference and click begin course.
    4. After you complete the attestation, return to your member dashboard to fill out the evaluation form (see below)
  1. Complete your Evaluation Form
    1. In your member dashboard
    2. Under “Evaluations and Certificates” click on “Training Evaluation”
    3. Find the evaluation that corresponds to the conference
  1. Print your Certificate

1.    Go back to “Evaluations and Certificates”

2.    Click on “Credits Earned Transcript” and print your CE certificate
